All images will come from Sweet and Talented.com except for the texture (I made that) I will keep my opinion out of this tutorial =D

First add your texture. You can make it or get one, I think I did both (Haha, been a while since I made this)

Next create a blend of Kristen Stewart (this this this this) and add to the top of the layout. Erase the bottom and set layer to Hard Light.
Duplicat the blend, add it to the bottom, and set it to lighten.

Then get this picture, resize it to fit better with the layout, and set it to multiply.

Next, I got this picture, and erased everything but the grass part, and added it to the bottom. You may have to duplicate it a few times and move it round to get the similar effect. Set this (or these, if you have multiple layers) to multiply.

Now, get this picture, and positon it like so, or get creative and move it somewhere else. Set it to multiply as well.

Next add this image, and erase everything but her head. Set this layer to Lighten Only, and be sure to erase any areas as you see necessary.

Finally, I added two more images to the top between the blend and set to lighten. And I added another one to towards the bottom, set to Lighten Only.

If you want, you can change the layout up completly by change in the texture used.
